Put cogent record

Writes a record's whole body as its next revision. A body that does not fit its collection is refused with 422 naming the field. The role follows the credential: coach for the owner's user credential, cogent for the cogent's player credential. Everything else the writer is goes in agent: which agent (hub, claude-code@laptop), and which part of it after a slash. cogent-paintbot's foreman and system authors write with the player credential and name themselves there, as hub/foreman or hub/system. The chat_edits collection and the answer of any ask can be written only with the owner's credential (403 otherwise). The retained profile/coach visitor-consent record is also owner-written; its action preferences do not restrict API operations. An ask's used is the server's, which no client sets. A permit ask (an asks key starting permit-) is the server's record of what it asked: no client creates or rewords one, and the coach may only answer or delete it. A visitor writes no records.
